Bring a question. Expect a clear answer.

# Bring a note, not a diagnosis.

A few honest notes about your concern can keep the conversation focused, even when you feel tired or unsure what to ask.

Assessment

The conversation and review used to understand a concern before deciding on a plan.

Begin with what is bothering you

Write a short note in your own words. Describe what changed, what you hoped to get help with, and what prompted the sermorelin search.

There is no need to turn that note into a diagnosis. Start with what you have noticed and what matters in your daily life.

A question to take with you

What else could explain this, and what would help us tell the difference?

Bring context, not a polished story

List the medicines and supplements you take, with their names as written on the labels.

Bring any records or test results you want the clinician to consider.

Write down earlier reactions or problems with care that you want to discuss.

Note pregnancy, pregnancy plans, or breastfeeding if relevant to your questions.

Include the ad or claim you want explained.

If you do not know a detail, write that down too. Ask how to fill the gap rather than guessing.

You can read the note aloud or hand it over. Ask the clinician to repeat anything that is unclear.

Bring your own words. There is no script to get right.

Leave with an answer you can repeat

Before the conversation ends, try saying the plan back in plain words. Ask the clinician to correct anything you misunderstood.

What problem are we assessing or treating?

Why this plan, and what other options did we discuss?

What needs to happen before a decision?

Who explains the results and answers later questions?

What have I agreed to pay?

Ask for a written copy of the plan and the name of the person responsible for follow-up. Leave any unresolved question on the page.

A useful plan says how the next conversation happens.

This guide helps you question a sermorelin care offer; operating clinics and selling medicines are outside its role.
